13. Identifying, reviewing and analyzing pertinent facility policy and procedure manuals, job descriptions, competencies reports, equipment manuals, equipment maintenance and repair logs

This service will identify pertinent manuals from the hospital, clinic or departments and other related facility documents to verify the treatments staff can provide and compare this to the treatment provided in the medical records.  Treatment that can be provided vary from department to department, this information would be noted in the unit policy and procedure manual. The policy and procedure manuals serve as the established standard of care for the department or facility; therefore, these references can determine deviations in care provided.

This service also locates:

• Job descriptions for all personnel involved in the alleged incident,
• Competency reports for all personnel involved
• Copies of equipment manuals for equipment involved in the alleged incident

• Copes of the equipment maintenance and repair logs  

A report summarizing the findings is prepared
